Action Health Partners (AHP) offers leadership training in all three Chronic Disease Self-Management Education (CDSME) programs – Chronic Disease, Chronic Pain, and Diabetes Self-Management – in English and Spanish.

About our Chronic Disease Self-Management Lay Leader Training

• Trainings are in English and Spanish for most programs
• Each training is 32 hours, divided into four days
• AHP covers the cost of the Leader Certification
• AHP provides food during training
• AHP provides materials including the Leader's Manual
• After being trained leaders receive $200 per workshop taught​
• To maintain certification as a Lay Leader, individuals will teach one workshop per year after they have received the certification

• Diabetes Self-Management
• Manejo de la Diabetes

• Tomando Control de su Salud - Chronic Disease
