BECOME A CCO
At Action Health Partners, we work with our partners to provide Medicaid and Medicaid/Medicare dual eligible beneficiaries with Health Home support throughout North Central and Eastern Washington. The Washington State Health Care Authority gathers data on potential clients, assesses their needs for the Health Home program, and shares their contact information to Qualified Health Home Lead Organizations, like Action Health Partners (AHP).
The Health Care Authority contracts with designated “Health Home Lead Entities” to provide Health Home services directly, or through contracted Care Coordination Organizations (CCO). AHP is a delegated lead for Region 6 delivering Health Home services since 2013. As the Lead organization, AHP contracts with CCOs that employ Care Coordinators to provide one-on-one services and supports to clients.
