Blood Pressure Monitor Check-Out ProgramLibrary cardholders, over the age of 18, can check out a blood pressure monitor kit for up to two weeks to self- monitor their blood pressure.

Chronic Disease Self-Management ProgramThe Chronic Disease Self-Management Program supports people with different or multiple chronic health problems. Program is led by trained and certified peers with first-hand experience. We discuss techniques on how to deal with frustration, isolation, appropriate exercise, fatigue, pain, medication management, emotion management, and much more.

Chronic Pain Self-Management ProgramThe Chronic Pain Self-Management Program was developed for people who have a primary or secondary diagnosis of chronic pain. Pain is defined as being chronic or long term when it lasts for longer than 3 to 6 months, or beyond the normal healing time of an injury. Program is led by trained and certified peers with first-hand experience. We discuss dealing with frustration, fatigue, isolation, poor sleep, appropriate exercise, appropriate use of medications, and much more.

Diabetes Self-Management ProgramThe Diabetes Self-Management Program supports people with all types of diabetes. Program is led by trained and certified peers with first-hand experience. We discuss techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, and all of the emotional problems associated with this disease. Our material has been reviewed by physicians, diabetes educators, dietitians, and other health professionals.

Healthy Heart Ambassador ProgramIn partnership with the Wenatchee YMCA, and with the support from the Washington State Department of Health, Action Health Partners is offering the Healthy Heart Ambassador-Blood Pressure Self-Monitoring Program. The 4-month program offers guidance from a trained Healthy Heart Ambassador. Participants will measure and document their blood pressure, attend two individualized meetings every month and one monthly nutrition education seminar to establish new and heart-healthy goals.
